Job Description
Job Title: Support Specialist- Residential
Department: Residential
Supervised by: Residence Manager
Salary range: $9.00/h - $10.45/h

Job Function: To provide care and supervision of the residents, carrying out recreational activities, activities of daily living, personal care, housekeeping activities, socialization and the implementation of individual program plans of each resident using verbal, and manual signing.

Responsibilities:
  • Develops and maintains positive interactional relationships with each resident conducive to promoting healthy growth and development. 
  • Provides individual and group supervision to residents and accounts for all residents under his/her supervision at all times. 
  • Directs and physically (when necessary) assists residents in their daily living routines and prescribed program plans, promoting individual growth through independence.
  • Leads and participates in recreational and leisure time activities.
  • Directs residents and physically participates in maintaining the cleanliness and the upkeep of the house, reports needed repairs to the supervisor.
  • Dispense and record medications prescribed to residents, reports medical problems through the appropriate channels and follows medical directives.  Maintains medication administration status.
  • Participates in the treatment team process by functioning as an advocate and/ or a backup advocate for a resident.  Provide treatment team objectives and methods via team meeting participation and/ or through communication with the Active Treatment Specialist QMRP.   
  • Implement treatment methods and objectives with each individual resident as prescribed in their program plan.  
  • Serves meals on time and prepares them when the cook is not available, following all applicable policies and procedures.
  • Transports residents to community activities, recreational activities, etc. when requested.
  • Implement behavior management plans of each individual resident when appropriate and/or indicated.
  • Maintains daily documentation in goal books, communication logs, resident travel logs, vitals books, reporting significant occurrences to supervisor and completing additional reports if needed, i. e. incidents, and/or reportable events.
  • Performs other similar duties requiring comparable skills as assigned by the supervisor.
  • Adheres to all agency policies and procedures. 
  • Advises supervisor of any obstacles to achieving the responsibilities described in this job description.
Qualifications:
  • Must be at least 18 years of age.
  • High school diploma or Equivalency (GED. 
  • Reliable transportation(NYS driver's license and be a licensed driver for 2 years and good driving record required for all positions.)
  • Ability to work independently and motivate others.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to lift at least 50 lbs. dead weight.
  • Maintain all required certifications/training by State regulations and CDS policy. 

CORPORATE QUALIFICATIONS:

  • Adhere to all CDS, Inc. policies and procedures.
  • Attend mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.
  • Act as a professional representative of CDS, Inc. in regards to appearance, behavior, temperament, communication, language and dress.
  • Perform all other duties relevant to the position as assigned by supervisor.
