Support Specialist - relief

Job Description
Job Title: Support Specialist- relief
Department: Residential
Supervised by: Residence Manager, Assistant Residence Manager
Salary range: $8.38 per hour

Job Function: An on-call or as needed basis in absence of full-time Support Specialist.  To provide support services to adults with developmental disabilities.  These services shall include the promotion of continuous learning and individual growth within Day program and the community.  Opportunities shall be based upon the participants interest areas andprogram plans.

Responsibilities:
  • Provide meaningful and functional activity-based programming consistent with individual program/support plans, personal vision, and the mission and vision of the program for all assigned participants. Give input to the treatment coordinator regarding any necessary IPP/ISP changes.
  • Ensure a safe, organized, clean environment that is conducive to learning.  Inform supervisor of any related concerns.  Complete work order as needed.
  • Serve as an appropriate role model and engage in interactions, which promote and insure the dignity and rights of all participants, service providers, co-workers, and community members.
  • Promote continuous learning for self, coworkers and customers. 
  • Accurately communicate and complete all documentation for individual program plans, reportable events, and incidents within established periods.
  • Ensure necessary resources are available as needed to complete scheduled activities.
  • Monitor status of all adaptive equipment, augmentative communication devices, module materials, and individual belongings.  Communicate need for replacement/repair to treatment coordinators.
  • Supervise community outings/volunteer placements according to the needs of the program.
  • Provide behavioral supports consistent with behavioral management plans, positive approaches, and SCIP-R techniques as needed.
  • Ensure customer satisfaction by doing what is necessary to meet the customers expectations.
  • Perform any other related duties as required by the treatment coordinator or day services management.
Qualifications:
  • Must be at least 18 years of age.
  • High school diploma or Equivalency preferred
  • Reliable transportation (NYS Drivers License preferred)
  • Ability to work independently and motivate others
  • Ability to communicate effectively, both orally and in writing
  • Ability to lift at least 50 lbs. dead weight.
  • Maintain all required certifications/training by State regulations and CDS policy
  • Must be able to read and write English to a level of fluency and comprehension which enables the candidate to follow written direction on labels, charts, medical devices and other sources of client care and/or production information as well as follow, comprehend and appropriately act upon same if transmitted orally.

CORPORATE QUALIFICATIONS:

  • Adhere to all CDS, Inc. policies and procedures.
  • Attend mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.
  • Act as a professional representative of CDS, Inc. in regards to appearance, behavior, temperament, communication, language and dress.
  • Perform all other duties relevant to the position as assigned by supervisor.
